Returning students Required Forms:
General Health Form (online; one per student)
All 7th Grade Students (NC Department of Health: Vaccines)
If you have any specific health needs such as allergies (insect, food, etc. or asthma, etc.), complete addition forms needed by the nurse by clicking here. If you want your student to be able to take over the counter medication such as Ibuprofen or Tylenol, you will also need to fill out a for Medication Authorization Form.

(Elementary and Middle School Summer Reading is recommended, but not required. High School Summer Reading/Assignments are required.)

LOGO Design: The Carmel Christian School logos are designed by the Carmel Christian School Communications Department and are owned by Carmel Christian School for the sole use of the school. They may NOT be used or re-created for any purpose by anyone other than designated members of the school administration. All school uniforms must be monogrammed only by our uniform vendor, Lands’ End. Parents should not distribute any Carmel Christian School logo for any reason.
Online Uniform Swap
Carmel Christian School Online Uniform Swap, a closed Facebook group created BY CCS parents FOR CCS parents to buy, sell and swap CCS uniform items in a convenient manner. This Facebook group is accessible only to the parents and family members of Carmel Christian School students. Please request to join the Facebook Group and, once the request is accepted, you will be able to post and purchase items for sale. Please Note: Only CCS-approved items (logo and uniform items only) should be listed on this group. The ONLY approved logo for 2021-22 uniforms is our current logo found on the CCS homepage.
Student Parking Request Form
HS students will need to fill out this form in order to park on campus. If we have to limit parking due to spaces available, priority will be given in the following order: Seniors and submission date, Juniors and submission date, sophomores and submission date, Freshmen and submission date. Families will be notified of final decisions prior to the start of school.

Painting your parking spot is like having a customized welcome mat for you and an opportunity to spread a positive message to others! Click here and read the document carefully as there are many important details to consider. Designs to Mr. Siftar and payment on Carmel Cashier must be submitted by Monday, August 1.
Student Request for NC Driver Eligibility Forms
The NC Driver Eligibility Certificate may only be requested upon completion of a Driver’s Education course. Students must provide the documentation given to them upon completion of their Driver’s Education course in order to pick up the Eligibility Certificate. Certificates expire after 30 days, so it is best to wait to request the certificate until the student has a confirmed appointment with NC DMV. There is a issue limit of one form per student. (Click here to complete the form.) Certificates take a minimum of 48 hours to process. Students will be emailed when their certificate is ready for pick-up in the main school office.
Athletic Workouts are continuing with modified processes and formats in place to uphold recommendations provided by the NCDHHS and our state athletic association. Click here to observe the summer Campus Health and Safety Protocols. All CCS student athletes must have current athletic forms (student physical and ImPACT Test-New HS student-athletes) on file PRIOR to trying out for any team.
